According to Ohio Law, we are required to have updated forms every year for your child on file. This year, all of the forms are required to be filled out online through Final Forms. Directions and the link on how to create an account to access your child's forms can be found on the school website under Final Forms. If we have an email on file for you, a link from Final Forms has been sent to that account. If your email has changed or you have not given the district an email, please call the number below.
If these forms are not completed by Tuesday, October 2, 2018, your child will be subject to disciplinary action. Notices were sent home on Wednesday with the affected students.
